We provide specialised care for people with a life expectancy of up to 12 months, helping them make the most of their time with loved ones. Our team helps manage symptoms, provide emotional and spiritual support, and coordinate care across all settings.


Our service is available to residents living within 40km of St Vincent’s Private Hospital Toowoomba.

Our Team

Experienced and Supportive Professionals

Our Palliative Care Team includes specialist nurses, doctors, and allied health professionals.

We work closely with patients, families, General Practitioners and treating specialists to help manage pain, relieve symptoms and enhance comfort and quality of life.

Community Palliative Care Service

Our Community Palliative Care Service aims to help patients live at home as actively as possible, for as long as possible.


Key features include:

  • 24/7 service availability for residents within a 40km radius
  • Symptom management support from a multidisciplinary team
  • Care addressing physical, emotional, social and spiritual needs
  • Direct hospital admission if acute support or end‑of‑life care is required
  • Ability to present to Emergency after hours for assessment
  • Some private health fund members (including BUPA, Medibank Private, NIB) may be able to claim costs associated with this service

24/7 Community Palliative Care Support

Our Community Palliative Care Team provides around‑the‑clock support through a dedicated hotline available 24 hours a day, seven days a week.
When patients are admitted to the service, they and their families receive a dedicated phone number to call whenever they need assistance or advice.

  • During business hours (Monday to Friday, 8am–4.30pm):
    If the team is unavailable and your call is diverted to voicemail, you can leave your name, contact number and details of your concern. A staff member will return your call as soon as possible.
  • Outside business hours:
    Calls are answered by our after‑hours paging service. A member of our clinical team will respond within approximately 20–30 minutes.

The dedicated hotline number is provided upon admission to the service.

Patient and Family Counselling:

    Patients and families can meet privately with our counsellors in a calm, supportive environment.

  • Online

    Access counselling through secure video sessions from the comfort of home.

  • ​Telehealth

    Speak with a counsellor directly for flexible, convenient support whenever needed.

Specialist In‑Hospital Palliative Care
Calm, Safe and Supportive In‑Hospital Care

Patients who need 24‑hour support can access specialist palliative care delivered within the hospital.

This service includes:

  • Close symptom monitoring
  • Pain and symptom management
  • Support from pharmacy, radiology and pathology
  • Coordinated care across the patient’s hospital journey
  • Where needed, we can arrange transfers to a service closer to home or another preferred location
