Registering for your hospital stay
The St Vincent’s Private Hospital (including Mater Hospital Sydney) Online Patient Admission Portal is a safe and secure online portal that provides patients the convenience of completing preadmission forms prior to admission to hospital. Please complete your online admission before your planned hospital admission date.
What you’ll need
- When you enter the portal, you will be guided in answering the Admissions Form and the Clinical History Form.
- Please ensure that you complete both forms (the Admissions Form and the Clinical History Form) at least 5-7 business days prior to your admission date.
- This pre-admission gives the hospital all the information it needs to process your stay and will allow you to securely manage your hospital admission online, communicate with the hospital and have your details saved for future admissions.
- After completing the process, you will receive a confirmation email that we have received your preadmission.
The following information is required, along with access to your registered mobile phone, to complete your Online Admission
Your current Medicare, pension or pharmacy benefit card details.
Your private health insurance, DVA or work cover details.
Your list of medications both prescribed and supplementary, the strength, dose and the reason for taking.
Your personal and next of kin details, your medical and surgical history and enduring guardian and/or advanced care directive information.
Depending on your medical history, registration can take approximately 15 to 45 minutes
Please be aware we will be asking you additional questions in the infection control section of Online Admissions due to the outbreak of the COVID-19 virus. It is important that you answer them accurately.
