Clinical Documentation & Coding

The quality of information in the patient's medical record is key to ensuring they have a safe and positive experience whilst in our care. Gaps in our clinical documentation impacts our inpatient revenue and our ability to resource and reinvest in improving the services we provide to our patients.

Where clinical documentation is not specific or diagnostic terms are not linked to treatment and findings, you may receive a documentation query which we ask is actioned within two days. Where a response is not received within this timeframe, the clinical documentation query will be escalated to the Hospital Executive Team.

Our Clinical Coding Team is available during business hours, Monday to Friday, to support you with any clinical documentation questions, particularly at the time of your patients' admission to ensure documentation is specific and complete. To contact the team, call 07 3326 3565.
