Palliative Care at Home

St Vincent’s Private Hospital Brisbane Palliative Care at Home

Compassionate, specialist care designed to help you maintain your quality of life in the comfort of your own home.

Brisbane Palliative Care

For many people living with a life-limiting illness, being cared for at home means remaining close to family, familiar surroundings and the things that matter most to them.

Palliative Care at Home is part of St Vincent’s Bringing Care Home program, extending our specialist care beyond the hospital and into your home.

Our Brisbane community service provides specialised care through a multidisciplinary team of clinical nurses and doctors, physiotherapists, occupational therapists, social workers and counsellors. We work closely with community General Practitioners, treating specialists, St Vincent’s hospital teams, domiciliary nurses and social services to provide coordinated, continuous care.
